Please note that incomplete applications are not subject to the SSMRCA’s review timelines.

To submit an application form either complete the application form online,

Alternatively print a copy of the form and complete it manually, and fax, mail or submit the application and additional information at the SSMRCA office.

All applications submitted require additonal information (see application checklist) to be submitted prior to it being reviewed.

Applications must be signed prior to an approved permit being issued.
